    Please fill out the information below. If you prefer, you may DOWNLOAD THE FORM and submit it to the Catechetical Ministry Office with your payment.
    Sacramental History: For each child below, please select the corresponding button(s) to indicate which Sacrament your child has received. Please provide a copy of your Child(ren)'s Birth AND Sacramental Certificates to the Office.

    **When completed filling out the form, click on the Submit button below. After clicking Submit you will be directed to our secure site to pay online where you will scroll down until you see Registration -Faith Formation to make a one time payment by filling in the required information.**
